This proforma is to be used for all personnel listed as working on an OGTR project at Deakin University. The form needs to be completed by the new investigator and signed by the Principal Investigator. Please submit completed form(s) with the project application (or a covering letter if the new investigator is to be added to an existing project) to the Secretary of the LBC. If you have any queries please direct them to the Administration Officer, Biosafety & Animal Ethics (AWC & LBC Secretary).
